The following links are to the standard U.S. immigration-required forms. They are in Adobe Acrobat format, and can be filled in on your computer. Unless you have the Adobe Write application, however, you cannot save the data for future editing, but must print it out.

In some cases, there may be later versions of these forms available from INS. Unless there are content changes, however, these versions are suitable for submission.

These forms, though prepared from original INS PDF-format documents, must be used at your own risk, and the moderators of MAG-ANAK can take no responsibility for any delays resulting from INS non-acceptance of forms you submit. You are advised to check the dates on the bottoms of these forms against possible later versions. Where applicable, appropriate colors and printing order is described.

G-325A, Biographic Information, 4 pages. Fiancee must sign all copies. Best to use pc-fillable, all white version. 07/14/2006 form date

FC-029, Important Document Information, to be included in any petition to USCIS if photocopies, rather than original or certified copies, are included (birth certificates, divorce papers, etc.). This is for USCIS Service Centers and offices only. The statement can also be inserted in a cover/transmittal letter and this form excluded. There is at least one other form (different form number) in use with the same language and same function.

I-130, Petition for Alien Relative (7/2007 version), 2 pages

K3 receipt Notice of Action is the first Notice of Action sent from INS informing petitioner that application has been received and file opened

K3 approval Notice of Action is the second Notice of Action sent from INS informing petitioner that application has been approved

K3 embassy letter is the instruction letter sent from embassy notifying petition-approved K3 applicant of embassy package requirements. This letter might be outdated.

I-864A, Contract Between Sponsor and Household Member, 3 pages, 01/15/2006 version. Used for spouse or household member acting as co-sponsor.

I-864P, Official Poverty Level and Income Reuirements standards for sponsorship eligibility, 2006 version

DS-230, Parts I and II, Application for Immigrant Visa and Alien Registration (7/2004 version), Part I to be filled out and signed, Part II to be filled out but not signed, and included with NVC packet, 4 pages.

Authorization, Authorization for Release of Federal Tax Data, recently added to the NVC package, to give NVC permission to request your tax information from IRS. This form comes with a bar-coded and readable Manila number (removed from this sample)

Legal Capacity Affidavit, A fascimile of the Affidavit of Legal Capacity, required to be sworn at the US Embassy or Cebu Consulate by American citizens planning to marry Filipino(a)s in the Philippines. Included also is an information sheet describing this requirement. Note: The info sheet is dated 1997, and some of its data may not be current.

Consent Form, A standard parental advice/consent form, used in Cebu, but probably standard enough verbiage for any area of the Philippines. For certain age ranges, this will be required for marriage in the Philippines. This is two pages, overlapping from a scanned form.

Marriage Application, Application for Marriage License, used in Cebu. Probably standard enough so you know what to expect anywhere in the PI. This is two pages, overlapping from a scanned form.

ADJUSTMENT OF STATUS

I-485, Application to Request Permanent Residency or Adjust Status, 4 pages, this is the 10/26/2005 version.

Supplemental Form to I-693 (Appendix A), Used by USCIS-approved doctors (Civil Surgeons) to certify shots for Adjustment of Status package

I-765, Application for Employment Authorization, 1 page, 2/08//2007 version

Military Retiree booklet, Military retiree resources, organization names, phone numbers, etc., recently compiled

I-693, Medical Examination of Aliens Seeking Adjustment of Status, only for applicants for whom petition and adjustment of status are initiated in the US because applicant is already in the US (not needed for spousal and fiancee visa holders). 8 pages, printed back-to-back, head-to-head, colors white, light blue, light green, pink, 2 consecutive pages of each color

I-131, Application for Travel Document, used to request Advance Parole, or approval to re-enter the United States if green card has not yet been issued, 3 pages. 4/26/05 version.

AR-11, Alien Change of Address Form, 2/20/2004 version. Must be submitted to Kentucky whenever non-citizen changes address.

I-751, Petition to Remove Conditions on Residence, 08/01/06 version, 2 pages, must be submitted 3 months prior to expiration of conditional green card.

W-7, Application for IRS Individual Taxpayer Identification Number, white, 1 page to fill out (this is official IRS fillable-version), and two pages of instructions. 12/17/2003 version. Used to obtain a Taxpayer Identification Number (TIN) for someone who is not yet eligible for a green card or is overseas without an approved visa. With a TIN, the holder can be claimed on a joint tax return (if you are married), but use of TIN renders the tax return ineligible for the earned income credit (for low-income folks). MUST BE SUBMITED WITH TAX RETURN.

NOTE: The TIN can be applied for directly in Angeles City thru: Jim Boyd, USAF, RAO Director & US Embassy Warden, Clark Air Base Region, Fax/Tel 63 45 888 5079/2748 This is an official IRS receiving station and the form is on their computer.

DS-3032 Choice of Address and Agent -- this is sent by the NVC, after spousal petition (I-130) is approved by the Service Center, as a prelude to NVC sending its forms to petitoner and beneficiary. The version sent by NVC has barcode and MNL number printed on it.
